    HTM Community,

    We will be using this focused discussion thread to discuss all things related to COVID-19. There is a lot of excellent work happening in the HTM field in response to this pandemic and this forum is a great place to share all your best practices and information that could be helpful to your peers. This forum can also be used to ask HTM COVID-19 related questions to your HTM peers. We are all going through this challenging time together and no one should feel like they are going through it alone. I hope everyone finds this discussion thread useful and will take advantage of it. Please do not hesitate to reach out to me directly if there is anything more AAMI can do to help. This discussion thread is open to everyone in the HTM field and you do not need to be an AAMI Member to use it. If you know someone who would like to be part of this group and is not, please have them email MaryJane Thomas at MThomas@aami.org.

    For our organization the HTM Department is heavily involved in assessing inventory readiness, equipment planning for expanded clinical areas, repurposing medical equipment, sterilizing/high level disinfecting PPE and acquiring quotes for purchasing/renting medical equipment.
     
    I am sure there are many other HTM Departments that are doing just what we are doing, but it has been brought to my attention there are departments that are trying to figure out what they should do. So, I propose AAMI and this MESSAGE board be used to share ideas and information that others can use. It is time to share and it is important that any idea is a good idea and while there are ideas that may be better it is important to share the ideas.
     
    Our department has been tasked with developing a medical equipment list that can be used to outfit expansion of specific clinical areas. We have put this spreadsheet together for items that we feel are required and items that we feel are desired. Some items were purposely left off because supporting ancillary departments would provide the service and equipment.
     
    I hope this is appropriate for this message board and that those who looking for information find this spreadsheet of value. It is not perfect, but it is a start. Sure would like to hear from New York and Washington.

    Scott,

        I agree that I would like to see more from other HTM Departments, but I disagree that AAMI needs a separate discussion group.

     

    Create a thread, we can all visit it. It's up to us, not AAMI. AAMI has already given us the tool to use as we need. If we as the HTM Community are not open to using this, then it is our fault, not AAMI's.

     

    I will ask this of Steve Campbell and Danielle McGreary and AAMI, can we open the door for this tool to non-AAMI members for the COVID crisis for exchange of information. I would propose that Non-members are a vital link of sharing information at this time. They may not have access to organize a resource on their own.

    Thank you Danielle and AAMI for providing this medium for HTM professionals to communicate on this pandemic.

    My HTM team is working to provide our hospitals across the United States with all of the possible tools necessary to provide our patients in the communities we serve with an excellent experience.  As mentioned, equipment planning and sourcing as well as ensuring disposable supplies used on the medical devices, such as ventilator connectors, is something our teams are helping our hospital/health system leadership with on a daily basis.  Our HTM leaders have worked with our Respiratory Therapy leadership to test and validate the use of a single ventilator on potentially more than one patient.  Our HTM leaders are engaged with our Incident Command teams to ensure we all have our "finger-on-the-pulse" of potential technology needs.  Is/Are our medical gas supplier(s) alerted to our potential needs for more frequent gas deliveries should we see a surge in patients (oxygen use could escalate very rapidly).  Our HTM teams are engaged in the "cleaning" process/reprocessing of N95 masks and other supplies for reuse by the caregivers.  UV-C and VHP processes are in use at our facilities to accomplish this reprocessing which was accomplished by working with our UV-C and VHP device manufacturers and professional published data such as the University of Nebraska papers and Duke University documented processes.  I could go on and on how our HTM leaders in our corporation are assisting with this pandemic and serving our patients.  They have risen to the challenge and are meeting the needs.

    One frustration that we are having is with CMS and some OEM's.  We now are experiencing some OEM's/vendors who are refusing to perform contracted planned maintenance services.  CMS has not published any waiver, that I am aware of, that relieves hospitals from the regulatory aspects of the medical equipment management plan, especially on those devices that CMS demands that procedures of the OEM must be followed.  These are some of the same vendors that have been pressuring the FDA demanding that ONLY manufacturers should be servicing the devices they sell to healthcare providers.  The irony is maddening that in a time of crisis, these OEM's will not support their customers, contracted customers no less, and meet the needs of the imposed regulatory requirements, i.e. planned maintenance.

    ​Thanks for the spreadsheet Steven, it's very useful. My department is also heavily involved in logistics and acquisition during this time. I've connected with the University of Wyoming who is printing hundreds of Billings masks and face shields at no cost to us. The institution is happy to spend their academic budget doing this since they have no students ATM. Our providers have asked for aerosol boxes similar to this: COVID-19 Aerosol Box so we're building those this weekend. I've also attached a spreadsheet I gave to our incident command center. It shows patient monitoring capability of each bed, a tool to help them decide where to direct the flow when necessary.

    Thanks Chris!
         I will join you on your request for clarity from CMS. We are, as an industry, way too busy moving fluidly through organizations servicing down equipment and assisting in deployment of reserve equipment to effective administer preventive maintenance at this time. I would hope that CMS could understand the validity of the request.

        I personally have not received notification from a vendor that they are going to refuse to provide service. It is their responsibility to make themselves a resource of support during a national emergency such as this. I would hope  that AAMI or FEMA, or some portion of the government could speak with all companies in this effort to be available to the healthcare industry as needed.

    Ventilator Filter Reprocessing

    As ventilator filters become more difficult to source, I was wondering if anyone has any data to support reprocessing of these filters.  Currently, our Supply Chain team is reporting backorders as far out as October on certain filters.
